Extended Health Benefits

With your consent, our clinic can set up Direct Billing to your extended health provider (provided it is one of our listed partners below) to save you the hassle and financial strain of seeking reimbursement yourself.

IMPORTANT REQUIREMENT: Although our clinic is happy to provide direct billing to your extended health provider, we do require that our direct billing form is filled out prior to the service being billed through our system. This form requires a Visa or Mastercard be placed on file with us in the event that the provider places a hold or refuses payment. This direct billing form is scanned into your secure online profile then shredded.

1. What if my insurance company is not listed above?

If your insurance company is not listed, we will require payment upfront and then provide you with a receipt. You may then submit that receipt to your insurance company for reimbursement. 

 

2. How do I check if I am covered by my extended health plan?

You have three options when checking your coverage 1) Read through your benefits booklet to check your coverage. 2) Log in to your insurance company's website to check your coverage. 3) Call your insurance company to check your coverage.

3. What questions should I ask my insurance company?  

Three great questions to ask your insurance company are 1) Is the therapy covered? 2) How much coverage do I have per year (dollar amount)? 3) What percentage of the fee is covered?

   

 4. What if I don’t have a Visa or Mastercard to fill out your Direct Billing Form?

If you do not have a Visa or Mastercard to complete our Direct Billing Form, then we will require payment up front and will provide you with a receipt. You will then be able to submit to your  extended health provider for reimbursement.
